In describing the correct set up, I think you tell us that club should form a “Y” between our arms.
How do we account for the forward shaft lean, especially in the higher lofted clubs, which forces our hands forward ahead of the ball?

Ed, the forward shaft lean is occurring because the golf club is not at the widest point of the arc at impact. The widest point is approximately 2 feet after impact with irons and three feet after impact with woods. If you are trying to make the club lean at impact you will tighten up the wrists and not hit the release point. you will still be holding on to this angle long after the ball has been hit.
To get forward shaft lean you have to keep the wrist loose and focus on rotating your body to hit the shot. Please watch this video to get a better understanding of the release point after the ball is hit:

When I started golf I had back problems and avoided taking divots. No watering systems in those days. Now I still do not take divots. Could lack of extensions be the problem? How important is it to take divots?

If you are no taking a divot you are hitting with your arms.

Paul ,When full y adhering to THE POWERLESS ARMS concept and couple it with VERY LOOSE WRISTS (I compare m’y grip on THE handle as if holding warm spaghettis ),and imagine you are GREG NORMAN with his very wide swing ,you get THE maximum arc width possible (Though Many purists said GREG NORMAN’s swing was too wide !)
Many purists don’t know what they are talking about. When did Greg Norman play his worst golf. ANSWER: When he flattened his swing. Not soon after he retired. Try telling Jack Nicklaus or Tiger 2001-2003 that his arc was too wide.
